CHR has hired a team of experienced individuals to meet the needs of individuals assigned to our clinic. Many of our team members such as housing specialists, employment specialists and case managers have over 10 plus years' experience working in SMI clinics. Our Psychiatric Nurse Practitioners have worked in behavioral health for many years to include ACT team and residential treatment providers. They are highly qualified and motivated to meet the needs of individuals they serve.
CHR is excited to combine the core principles of Consumer Operated Programming in an Outpatient Behavioral Health Home Clinic. The experience of members will be different with a high focus on member input and empowerment while providing the highest quality of care possible.
In November of 2024 , CHR launched it's Outpatient Behavioral Health Home.
This program offers Supportive and Connective case-management services as well as Navigator services to individuals who experience Serious Mental Illness as assigned through the Regional Behavioral Health Agreement in Maricopa County.
