Frequently Asked Questions About Services at CHR
To receive service at CHR, you must be an individual who has a behavioral health condition such as mental health and or substance use disorder.
If you have been deemed SMI your clinic must refer you to CHR. A referral must be sent from your case manager.
If you are not associated with a clinic or do not have SMI designation, then any provider can refer you to our services or you can self-refer. CHR will make sure we have the documents we need in order to allow for services to begin and continue.
No, CHR is an independent adult program. You must be 18 years or older to be here at CHR.
After you have completed the intake process, you are allowed to bring a guest/someone who supports your recovery once a month. You must have 24-hour approval for your guest.
If you have a friend or family member who might benefit from our services, we will issue a visitor pass for the individual to allow us time to see if an intake into programming is possible.
You must have completed an intake and be a CHR participant to attend any of the offsite activities. It is not appropriate to have friends or family meet you off-site unless it is otherwise planned as part of the outing.
Transportation services are set up by your case manager at your clinic or using the transportation portal. Please call us to find out more.
CHR is not a clinic. we are a peer-run and peer-driven agency that provides peer support services. Participants are referred to CHR to help with their wellness goals through classes, groups, and one-on-one support with peer guidance. We work with clinics within the Mercy Maricopa Integrated Care System.
CHR has a variety of volunteer opportunities throughout the year and will customize volunteer experiences based on interests. Hosting a special art class for our participants, joining us at an event in the community, and helping us to share information about CHR, administrative duties such as helping us to assemble program materials, or even rallying your friends to donate unused personal hygiene items for our participants are just some of the ways you can get involved.
